What Is Khindivi
Khindivi is part of the Corticosteroids class and treats Adrenal Insufficiency. Corticosteroids are used to treat a variety of conditions including skin issues such as eczema and psoriasis, eye inflammation, asthma, gout, allergic conjunctivitis, ulcerative colitis, edema, adrenal insufficiency, and hemorrhoids. They work by blocking substances in the body that cause swelling. Khindivi is only available as a brand name drug.
This medication contains hydrocortisone, a corticosteroid, also known as cortisol. The chemical name of hydrocortisone is 11β,17α,21-trihydroxy-pregn-4-ene-3,20-dione and it has the chemical formula of C21H30O5, and molecular weight of 362 g∙mol–1. Hydrocortisone is a white or almost white powder soluble in the pH range of 1-7
Uses And Indications For Khindivi
For pediatric patients with adrenocortical insufficiency who are five years of age or older, this medication is recommended as replacement therapy.
Limitations of Use
Increased dosage during stressful or acute circumstances is not authorized. For stress dosage, switch to a different medication formulation that contains hydrocortisone.
Dosage And Administration Of Khindivi
Important Considerations for Dosing
- Individualize the dose for each patient, using the lowest possible dosage.
- Monitor patients for symptoms of under and/or overtreatment including signs and symptoms of adrenocortical insufficiency, linear growth, and weight gain. Adjust doses accordingly.
- When stress dosing is needed (during episodes of acute febrile illness, gastroenteritis, surgery or major trauma) use a different hydrocortisone-containing drug product.
Recommended Dosage and Administration
- The recommended starting replacement dosage is 8 to 10 mg/m 2daily administered orally with or without food. Higher doses may be needed based on the patient’s age and symptoms of the disease. Use of lower starting dose may be sufficient in patients with residual but decreased endogenous cortisol production.
- Round the dose to the nearest 0.5 mg or 1 mg.
- Divide the total daily dose into 3 doses and administer 3 times daily. Older pediatric patients may have their daily dose divided by 2 and administered twice daily.
- Administer the medicine using the oral syringe provided by the pharmacy.
- KHINDIVI may be administered through a gastric tube. Flush gastric tube with 20 mL of water to ensure the entire dose is delivered.
KHINDIVI should be stopped because of negative reactions linked to inactive ingredients
The inactive components of KHINDIVI, polyethylene glycol 400, propylene glycol, and glycerin, have been linked to hyperosmolarity, metabolic acidosis, hypoglycemia, hepato-renal damage, toxicity to the central nervous system, and severe reactions in the gastrointestinal tract. Monitor the patient and think about stopping the medication and switching to a different hydrocortisone product if the patient exhibits negative reactions that might be connected to these circumstances.
Changing from Other Oral Hydrocortisone Formulations to KHINDIVI
Use the same total daily hydrocortisone dosage when transferring patients from other oral hydrocortisone formulations to KHINDIVI. Keep an eye out for signs of adrenocortical insufficiency in patients after switching to KHINDIVI. Increase the total daily dosage of KHINDIVI if adrenal insufficiency symptoms appear after switching.
Dosage Forms And Strengths
KHINDIVI oral solution, 1 mg/mL is a clear, colorless to slightly yellow colored viscous oral solution.
